National Purple Heart Hall of Honor

ENROLLMENT

We are pleased to provide you with information regarding enrollment in the Roll of Honor database. This database provides information about each Purple Heart recipient’s story and it can include photographs and scanned documents which have been provided to us that help enrich/enhance his or her record.

The Hall of Honor is the nation’s sole repository dedicated to the preservation and collection of the stories of Purple Heart recipients. The mission of the Hall of Honor is to collect, preserve and share the stories of Purple Heart recipients in all branches of service and for all conflicts for which the award has been available.

Enrollment Documentation

To enroll a Purple Heart recipient in the Roll of Honor, please submit the following information:

  • Enrollment Form (pdf)
  • Verification of receipt of Purple Heart Medal
  • Please submit a copy of one or more of the following:
    • DD-214 (or DD-215 update)
    • WD AGO 53-55 (WWII)
    • General Orders or Permanent Orders (awarding the Purple Heart)
    • Purple Heart Certificate or other supporting documentation as appropriate.

To obtain a copy of the necessary documentation, please contact the National Personnel Records Center or by phone at 314-801-0800. You must complete the standard Form 180 (SF180) in order to obtain military records.

Supporting Documents

We are also collecting a photograph of each recipient as well as a short narrative describing how the recipient was wounded or killed. We can accept additional materials such as letters, newspaper articles and pictures that contribute to the recipient’s Purple Heart story.

  1. Click the icon in the lower right and print the form
  2. Complete the form and make a copy of it, or complete two forms
  3. Be sure your signature is on both copies of the form
  4. Include a copy of your supporting document. One from this list

MISSING MILITARY RECORDS AND MISSING MEDALS MISSING DD-214s & MILITARY RECORDS

Military Records are in the custody of the National Personnel Records Center (NPRC) in St. Louis.
To request a copy of a DD-214 or other records, an individual must provide:
– Veteran’s complete name used while in the service
– Service number
– Social security number
– Branch of service
– Inclusive dates of service
– Date and place of birth

DD-214s can now be requested online. The National Personnel Records Center provided the
following website for veterans to request their DD-214 online: http://vetrecs.archives.gov/.
Requester still has to mail or fax a signature page to NPRC to complete the process.
All requests for records must be signed and dated by the veteran or next-of-kin. If the veteran is
deceased, a copy of the death certificate must accompany a request.
Note that a fire at the Records Center on 12 July 1973 destroyed a major portion of records of Army
personnel who were discharged between 1912 and 1959, as well as Air Force personnel records for
the period 1947 through 1963 with surnames Hubbard through Z. If the veteran suspects that his or
her records may have been involved in the 1973 fire at NPRC, a request should also include; place
of discharge, last unit of assignment, and place of entry into the service.
MISSING MEDALS
One of three options may apply for a veteran who does not have his or her Purple Heart medal, or
another decoration:
1. The veteran did not receive a Purple Heart, believes he or she is eligible for a Purple
Heart, but military records do not reflect that he was awarded a Purple Heart. The
veteran must complete the Application for Correction of Military Record (DD Form 149, Sep
2007) and send it to:

The application must be accompanied by proof or evidence, such as statements of witnesses,
medical records, newspaper articles or other pertinent correspondence.
2. The veteran received a Purple Heart, or another medal, has lost it and wants it replaced.
The veteran must submit a request to:

3. The veteran has not received a Purple Heart medal, but believes his or her military records
include evidence of having been awarded the Purple Heart, or any other decoration. The

Isanti County Minnesota- Purple Heart Community ceremony April 17th

On April 17th, 2019 at the Isanti County Board Meeting a proclamation will be signed by Board Chair Mike Warring that Isanti County recognizes the sacrifices made by Purple Heart recipients.

We formally invite all Purple Heart recipients and their families to attend the board meeting in order to show support for the proclamation.

Please pass the invitation along to anyone that you feel would like to attend the event.
